Need help with Medals & Ribbons was created by TornadoADV

Just finished Darkest Dawn (After doing Prologue of course) and I seem to be missing some awards in my medal case. From what I can dig up on these forums, it's the Distinguished Service Cross, Starburst Medal and one Campaign Service Medal (Black, Teal and White).

Anybody have any help on how to earn these medals? (Also, I earned two Bronze Stars and two Silver Stars, is there a second Gold Star out there?)

There exist a Excel file. Paladin have uploaded this file to rapidshare, but it's gone now. But i saved the file at wingcenter.net, look here.
